  2. Items required for Health Certificate (Form B)
  3. Original Diploma and Transcript (unauthenticated version) with one photocopy
  4. Authenticated version of the Diploma and Transcript with one photocopy
  5. Proof of Valid Health Insurance for 6 months (If you have it)
  6. Student academic record entry form
  7. ID photo * 1
  1. Diploma and Transcript must be authenticated by the Taiwanese Representative Office in your country
  2. Both of the unauthenticated original version and authenticated original version must be handed in with its photocopies
  1. After OIA check all the required documents, you need to go to Academic Office to complete the registration.
